League Registration
Registering your League as an Adult Safe Hockey Network Affiliate is as easy as 1-2-3...

3 - Submit the form by e-mail, fax or mail.
We will contact you to confirm: receipt of your application, payment of all applicable fees, and delivery of necessary documents, supplies, and prizes. In the meantime, should you have any questions about the ASHN Affiliate League Program or require assistance with the application form, do not hesitate to contact us.
Player Registration
All Affiliate League Players will be required to add their name, age, e-mail address, and signature to an Official Team Roster & Waiver Form. The Team Representative must keep the original on-hand (in case new Players are added after the first game of the season) and submit an up-to-date copy to the League Organizer. To ensure all Players are registered and eligible for coverage under the Sports Accident Insurance Policy, the League Organizer must forward a current copy of all Team Rosters by e-mail, fax or mail to the ASHN Office.
